*Please note: if your child is Year 5 or above then they are required to attend a minimum of two classes per week to get the best out of their HQ experience.
Fees must be paid on or before the first lesson of each half term. Interest at statutory rates may be charged on overdue fees or your child's place closed.

All medical information must be kept up to date.

Pupils up to and including school year 8 are not permitted to bring mobile phones or other electronic devices to the dancing school.

The school accepts no responsibility for pupil's personal belongings, whether they are lost or damaged while at the school.

The school phone is always available for pupils to use in an emergency.

Pupils up to and including school year 8 must be collected from inside the dancing school; pupils are not permitted to wait outside.

School dance uniform only should be worn for lessons - fashion exercise wear is not permitted.

A commitment to classes is expected; pupils should attend regularly, promptly and in proper uniform, with hair tied back in a neat ponytail.

Parents should regularly check notice boards and read emails for important information concerning rehearsals, examinations, term dates etc.

A call or text to the school must be made in the event of sickness.

Emergency numbers, addresses & schools must be kept up to date.

Dancing is a practical and artistic activity, and parents accept that reasonable physical contact may be necessary for correction of technique and style.

A minimum of one half term's notice must be given to terminate a place. Failure to give this may result in fees being charged in lieu.

If a holiday or unattendance is taken during term times the school is not responsible for refunding missed classes.

Please note that if you are under 18 years of age this form must be verified by your parent or guardian. To the best of my knowledge all the information provided on this application is correct.
